IMPORTANT TAX INFORMATION
In an effort to reduce paper consumption, the City will cease mailing paper sales and use tax return forms beginning January 1, 2016.
City of Lakewood sales and use tax return forms will be provided electronically via Lakewood Business Pro. In order to access these forms, taxpayers must create a user profile on Lakewood Business Pro. By creating a user profile, the user will have the ability to print their account specific returns forms as well as the option to file returns and remit payment electronically. If users do not have the ability to remit payment electronically, hard copy checks can still be mailed in.
Because City of Lakewood sales and use tax return forms are tailored specifically to each taxpayer account and filing period, blank forms will not be available on the City of Lakewood website or any other website. The information pre-populated on the account specific forms, including bar code and media number information, is vital for accurate internal processing of return forms.
For any businesses that do not have internet access, the City has provided a kiosk at the Revenue Division front desk where returns can be filed online.
